Parish Volunteer forms

By instructions from the Archdiocese of Sydney, any person over the age of 18 years who volunteers for the parish in one or multiple ministries, are now required to complete and return two very important forms.

If you volunteer in any capacity for our parish, please download and complete these two forms and return it to the parish office at your earliest convenience. Failure to return your completed forms to the parish office will mean that you cannot take up or continue with your voluntary work until the forms are received.

In response to the Royal Commission into Institutional child sexual abuse, the Archdiocese of Sydney has set up a Safeguarding and Ministerial Integrity Office which has developed protocol to create a safe and nurturing environment for children and vulnerable people.  The Archdiocese has now mandated that every adult volunteer within each of its parishes MUST sign a Code of Conduct.

This form (relating to Workplace, Health and Safety) is required to be completed by every volunteer at the start of each new calendar year.  

Please download and complete this form and return it to the parish office at your earliest convenience OR no later than 1 March each year.

NOTE: If you volunteer across a number of ministries – for example: sing in a choir, Commentator and an Extraordinary Eucharistic Minister, please check with the parish office first if you are required to complete a separate form for each ministry.
